Microscopic colitis: A long path to the diagnosis of inflammatory bowel disease at a young age (clinical case report)

Abstract. Inflammatory bowel diseases (IBD) represent one of the most important and unresolved problems in modern gastroenterology and coloproctology. Diagnosing IBD is challenging due to the absence of single pathognomonic feature and universal “gold standard” for diagnosis, and a problem of differential diagnosis between the main forms of IBD. Microscopic colitis occupies an important place in the classification of IBD. Its symptoms are nonspecific and the disease typically has a prolonged and indolent course. Difficulties in diagnosing microscopic colitis are associated with the lack of clinical suspicion in young patients, the absence of a clear diagnostic algorithm, insufficient continuity between clinicians and endoscopists regarding indications for invasive diagnostic methods, and the lack of macroscopic changes at early diagnostic stages during endoscopic examinations. As a result, diagnosis and appropriate treatment are often delayed for many months and years. This article presents a clinical case of a 35-year-old patient with microscopic colitis and describes the long outpatient diagnostic pathway.
