Peripartum cardiomyopathy with high arrhythmogenic activity: A clinical case
Abstract. Current article is dedicated to a pressing issue: decreased left ventricular (LV) ejection fraction in the third trimester of pregnancy, its risk factors, and causes. According to the literature and authors’ own data, the incidence of such cases has increased significantly in recent years. Much attention is being paid to the most common cause of this condition: peripartum cardiomyopathy (PPCM). PPCM, or postpartum cardiomyopathy, is an idiopathic cardiomyopathy characterized by heart failure (HF) secondary to LV systolic dysfunction at the end of pregnancy or within a few months postpartum, in the absence of other identifiable causes of HF. The prevalence, etiology, and pathogenesis of this disease are still remaining to be unknown for sure. The article presents a clinical case of a patient with PPCM, on the basis of which the discussion concerning its causes, possible risk factors, and treatment principles is carried out. In addition to a reduced LV ejection fraction, the patient had high arrhythmogenic cardiomyocytes activity, necessitating not only full standard drug therapy for HF but also close attention to cardiac rhythm control.S.D.
