Procalcitonin metabolites and macrocalcitonin in the diagnosis of hypercalcitoninemia of various origins
Abstract. Hypercalcitoninemia occurs in various tumor and non-tumor conditions and is not always associated with medullary thyroid cancer, complicating the interpretation of elevated calcitonin levels. This review examines current data on procalcitonin metabolites and calcitonin isoforms, primarily catacalcin and macrocalcitonin, and their potential diagnostic value in various types of hypercalcitoninemia. Special attention is given to the analytical limitations of immunometric methods for calcitonin determination, including the influence of heterophilic antibodies, the “hook” effect, and macromolecular complexes. It is shown that catacalcin may reflect active production of the hormone, while macrocalcitonin is one of the causes of falsely elevated calcitonin levels. Comprehensive assessment of laboratory parameters and record of potential analytical interferences can improve diagnostic accuracy and reduce the risk of medullary thyroid cancer overdiagnosis.K.Sh.
