Mönckeberg’s disease: A case report

Abstract. Mönckeberg’s disease is a systemic medial calcification of muscular arteries with calcification of the media while intima remains intact, which distinguishes it from atherosclerosis. This article presents the case of a 78-year-old female patient with widespread calcification of the coronary arteries, valves, aorta, and peripheral arteries of the upper and lower extremities. Coronary angiography revealed a multivessel lesion with severe calcification and a normal lipid profile, which could not be recanalized even with a rotablator. Echocardiography revealed severe mitral and aortic regurgitation with severe valve calcification, which made implantation of the MitraClip device not possible. Introduced clinical observation suggests that Mönckeberg’s disease should be considered as a possible cause in patients with widespread vascular calcification that does not fit the classic picture of atherosclerosis. However, due to the lack of specific markers, its diagnosis requires a comprehensive evaluation.
